Transition & Post-19 Progression: What Success Looks Like

To support families with transition planning, we use Future Plans built on from Year 7, helping students define their aspirations and next steps to ensure a seamless move into post-19 education, work, or care services.

We ensure that students and families have clear, structured pathways beyond Post-16. Our goal is to ensure that students transition successfully into adulthood, achieving outcomes therefore our provision directly links with:

  • Further Education Providers (SEN Colleges, Supported Learning Programmes)
  • Supported Internships, Volunteering & Work Placements– Tailored work placements .
  • Adult Social Care & Supported Living Services– Developing daily living skills .
  • Social inclusion and Community-Based Opportunities – Building friendships and community networks .
  • Health and well-being – Accessing appropriate healthcare and maintaining self-care routines .

CPA Sixth Form Pathway Destinations

 Accreditation Opportunities:

Accreditation opportunities are broad, challenging and meet the range of abilities of our students, whilst study programmes build on each learner’s prior attainment and enable them to make progress and move on to a higher level of qualification when they are ready to do so.

Pathway 1 students gain accreditation for Gateway in Maths and English Entry 1 to Level 2 and were appropriate and AS Fine Art.

Pathway 2 students will gain accreditation for ASDAN Life Skills Challenge for English and Maths, and where appropriate Duke of Edinburgh Bronze.

Preparing for Adult Life:
Social Care Support

As part of the planning for moving to adulthood, there are several options for young people with special educational needs and disabilities (SEND) to consider once they have left school. These can include:

  • Community-based learning, support and development opportunities
  • College placements on the pathway to employment
  • Apprenticeships or supported internships on the pathway to employment

Finding out about colleges

Trinity student Dawson and his Mum Victoria were among the parents and students who came along to chat to the colleges about the courses they had available.

Dawson is interested in going to Orchard Hill or Livability Nash. He wants to study film and TV and particularly enjoys animation such as Wallace and Gromit.

London South East College

London South East College provides outstanding education and training for young people with Special Educational Needs and Disabilities (SEND) across the boroughs of Bromley, Bexley and Greenwich.

They offer some great courses including:

Introduction to Work / Entry Level Award /Certificate Full Time 

The course is designed for young people with SEND who wish to develop the necessary skills for independence and for future employment. Find out more.

Personal Progression Pathways are individual learning programmes created to support the students’ needs and aspirations as they prepare for adult life. Find out more.

Community Health Professionals

The transition event was also attended by community health professionals like Daniella who is currently studying to be a Speech and Language Therapist at Greenwich University Medway.

She is currently undertaking her university placement at the Woolwich Centre.

Oxleas NHS

Oxleas offer a range of NHS healthcare services to people living in South East London including community health including care for people with learning disabilities, speech and language therapy, physiotherapy, mental health and more.

Index Day Opportunities 

At Index Day Opportunities, adults of all ages, with disabilities are supported to participate in a variety of fun and engaging activities (such as visiting parks, zoos, farms, trampoline parks and more) designed to enrich lives, provide creative outlets, whilst also developing life skills and developing self-confidence. They are based in Pugin Place in St Peter’s church in Woolwich and Charlton Assembly rooms in Charlton Village.

Greenwich Short Breaks

You may be able to get overnight short breaks if you have children who have complex health needs which may require care overnight or severe and profound disabilities.

They can also provide advice about activities available during the school holidays.

Brighter Futures Skills Hub

Based in Woolwich Brighter Futures Skills Hub assist adults with learning disabilities lead an independent and fulfilling life. It helps them to maintain a healthy lifestyle, and promotes and enhances effective personal support networks.

They are committed to helping individuals live safely and independently and be socially included within their local communities. They have some great activities to get involved with including cooking, arts & crafts, painting, gym sessions, yoga, disco evenings, music therapy sessions and more.

Trinity People

Trinity People is a SEND Day service, providing tailored activities for adult aged 19 to 30 years old. They aim to prepare adults with SEND for life by developing their communication skills and their knowledge and understanding of how to live their lives to their full potential.

They offer a range of activities including art therapy, arts and craft, music, cooking, independent living skills and lots more.
